COBRA/Retiree Benefits:

Effective July 1, 2024, COBRA/Retiree insurance with Eden Prairie Schools will be administered by Benefit Extras. Our contract with HR Simplified (aka Chard Snyder) will discontinue on June 30, 2024. In an attempt to decrease issues during the transition period, the District highly recommends that all June 2024 payments be made to HR Simplified no later than June 14, 2024.

In June, you will receive 2024-25 plan year coupons providing you with the monthly amounts owed for the upcoming plan year as well as instructions to set up a monthly automatic bank withdrawal of your premiums if you wish to utilize this option. All payments for July 2024 coverage going forward should be sent to Benefit Extras. Please watch your mail toward the beginning of June for your communication from Benefit Extras.

1095-C Tax Form:
If you are enrolled in district medical insurance - OR - you have a VEBA balance at any point in a calendar year, you will receive a 1095-C Tax Form to include with your taxes. These forms are mailed in March each year for the previous calendar year. You are able to file your taxes without it, but be sure to keep it with your records in case of future IRS audit.
