Summer School- 2008

Registration Form Information

Instructions for filling out the "Counselor Confirmation Form" - this is our registration form for Independent Study:

1. Fill in all information for the lines above "Course Title" - especially important are the lines "School Attending," "Grade" (NEXT school year - '08-'09), "Resident District #," and "Graduation Incentives" (the list to choose from is right next to this form).
2. Under "Course Title" be very specific. Please note that there is a maximum of 2 classes, and all A classes are from 8:00-10:00 and all B classes are from 10:00- 12:00.
3. Please leave the "CLP Goals" section blank.
4. It is very important that we know if your student is receiving Special Education, 504 or ELL services.
5. The form must be signed by BOTH the student and the parent. The form should be returned to Becky Brown at EPHS. She will get the Counselor's Signature.
